REGISTRATION FEE starts at $95 and will go up $10 up each month until August 2022 due at time of registration.

 We accept Cash, Check, and Credit Cards. 

All payments should be PAID IN FULL before a player will receive any equipment.

DOCUMENTS REQUIRED
for each player
- State ID or Military ID
- Report Card [end of year 2022.  There is a homeschool form at the field.  Kindergarten students do not need at this time]
- Copy of Birth Certificate 

FORMS MUST BE COMPLETED
Complete each of the following forms. 
Each player will need to have a copy of all 5 forms completed.
All of these forms are available for download or at the field at the time of registration.

> 2021 AYF Medical Clearance [Physical from Doctor]
        No child is permitted on the field for practice without a complete/current physical. 
        Moore Orthopedics offers free physicals.  Call them today.
> 2021 Player Liability Waiver
> 2021 AYF Player Parent Code of Conduct Form
> 2021 AYF Player Image Release
